Ordering a Test Kit
Patients interested in SIBO testing:
Please speak to your doctor as we must have a signed requisition form from a doctor in order to send patients a test kit. Your doctor can use the SIBO Testing Center Order form provided here.
Providers:
If you have prescribing rights, follow this link to fill out the Provider Registration Form. After verifying your credentials a lab requisition form will be sent to you in a follow-up email. Since the kit contains lactulose, we must ensure that you have prescribing rights to order the test.
The ordering process involves:

The provider faxes the completed requisition form for a breath test to 503-207-6212.

The patient submits payment online following the link located at the bottom of the SIBO requisition form.

When payment is received the test kit will be shipped to the patient.
How much does the test cost?
At-home breath testing kits cost $179 which includes shipping and processing fees. We do not bill any insurance.
Payment
When a provider recommends breath testing, they should direct the patient to the payment portal on the bottom of the requisition form. The SIBO Testing Center can supply the patient with a superbill to submit to their insurance company for reimbursement if requested.
Returning your SIBO Kit to the Lab
All kits mailed in the US will have a return shipping label. The completed kit can also be dropped off at the SIBO Testing Center Lab anytime the clinic is open.
Preparation for the Lactulose Breath test
Two Weeks Before Collection
Stop taking any antibiotics or antifungals (except those who are re-testing following Elemental Diet)
DO NOT undergo colonoscopy, colonics or barium enemas during this time.
DO NOT perform within 2 weeks of atypical diarrhea. If you have questions, please consult with provider.
4 Days Before Collection
Avoid all Proton Pump Inhibitors (PPIs) such as Omeprazole, Protonix, Nexium, Prevacid, and Prilosec
Avoid all laxatives. This includes (high dose) vitamin C and magnesium that is being taken specifically to induce a laxative effect.
24-48 Hours Before Collection

Avoid all non-essential medications and supplements including probiotics. If you have questions about what may be non-essential vs essential, consult with your provider.
The following list of foods are the ALLOWED foods for the preparatory diet:

Any meat, fish/seafood, poultry, or eggs

Plain, steamed white rice

Clear meat broth (no bullion, bone/cartilage broth, or vegetable broth)

Fats/oils (eg. Coconut/olive oil/vegetable oil, butter or lard)

Salt and pepper
-
Weak black coffee and/or weak black tea (plan, no sweeteners or cream)
-
Plain water, no mineral waters
Morning of Collection (after overnight fast)
Wake up at least 1 hour prior to beginning the collection. You may brush your teeth as normal
DO NOT eat, smoke, or exercise prior or during collection.
It’s okay to drink plain water during and before performing breath test
Read the instructions provided in the kit, label tubes and collect samples every 20 minutes
